To all Parents of 5th Grade students:

By NH state law all children entering the 6th grade are REQUIRED:
-to have received 2 doses of varicella (chickenpox) vaccine (history of disease is acceptable)
-to have received a Tdap booster (tetanus, diphtheria, acellular pertussis) if it has been 5 or more years since the last documented dose. (Except if your child turns 11 on or after the first day of school (September 4, 2012), they are required to have Tdap prior to first day of the next school year)
Please send written documentation to CLS Health office as soon as possible.
